Referral Process:

We currently receive referrals from various sources, such as primary doctor’s offices, therapy providers, schools, area colleges, and community mental health centers. We are excited to partner with other providers in our community and proud to be trusted with referrals.

To make a provider referral we need the referral reason, such as “testing for ADHD and anxiety” as well as the adult’s name and email address. For under age 18 years, we need the referral reason, child’s name, as well as the parent’s name and email address. Referrals can be faxed to our secure fax: 401-679-9289

The interested person can also reach out via email or on our website by sending a website contact form in.

Once a referral is received or potential client contacts the office, we will generate them our office consent forms. This comes via email so the individual can check their email for any communications from our office and details about next steps.
